Proof of service requirements
We want to be sure that our donations and resources are going to the right folks: those who have served or are currently serving in the U.S. military. All applicants are required to provide a copy of one of the following documents, without exception:
- Military ID card
- DD214 proof of discharge, if retired or veteran
Before filling out our application, please be sure you provide the documents requested. We need the documents and your application to determine your membership.
Sending your documents
Send the member Patriot application form and a copy of your military ID card or form DD214 in the way most convenient for you:
- By email to membership at petsforpatriots dot org. All documents must be in one of these formats: Word (.doc), text file (.txt), JPEG (.jpg) or PDF (.pdf).
